To obtain a Nuclear Pharmacist license in Florida, an individual must meet the following requirements pursuant to Section 465.0126, Florida Statutes, and Rule 64B16-26.303, Florida Administrative Code:

  1. Florida Pharmacist License: An individual must currently hold a Florida pharmacist license that is active and in good standing.
  2. Proof of Eligibility: The applicant must have successfully completed 200 clock hours of formal didactic training sponsored by an approved program or university. The training must meet the requirements set forth in Rule 64B16-26.303, Florida Administrative Code. This training must be certified by the university or approved program and will serve as proof of eligibility.
  3. Completion of a Period of Training and Experience: The applicant must have successfully completed 500 hours of training and experience under a supervising nuclear pharmacist. If the didactic and experiential training have not been completed within the last seven (7) years, the applicant must have been engaged in the lawful practice of nuclear pharmacy in another jurisdiction at least 1,080 hours during the last seven (7) years. The training and experience must meet the requirements set forth in Rule 64B16-26.303(2), Florida Administrative Code.

The following steps apply to individuals who currently possess a Florida pharmacist license that is active and in good standing and wish to work in the capacity of a nuclear pharmacist in the State of Florida:

  1. Apply for NUCLEAR PHARMACIST licensure. Ensure that all fields are completed as instructed. An incomplete application shall expire after one (1) year. Applications submitted without fees will not be processed. Please make sure to include your Social Security Number (SSN) on your application.   2. Complete 200 clock hours of formal didactic training sponsored by an approved program or university. The training must meet the requirements set forth in Rule 64B16-26.303, Florida Administrative Code. This training must be certified by the university or approved program and will serve as proof of eligibility. Please submit a certificate/letter documenting 200 hours of formal didactic training to the Board office.
  3. Complete 500 hours of training and experience under a supervising nuclear pharmacist. If the didactic and experiential training have not been completed within the last seven (7) years, the applicant must have been engaged in the lawful practice of nuclear pharmacy in another jurisdiction at least 1,080 hours during the last seven (7) years. The training and experience must meet the requirements set forth in Rule 64B16-26.303(2), Florida Administrative Code. The training and experience must be documented on the Certificate of Training and Experience Form and submitted to the Board office. The supervising nuclear pharmacist must sign and date the form.

Please allow 30 days for initial review of new applications. All applications and document submissions are reviewed in the order they are received. After your application is reviewed, any deficiencies will be communicated to you in writing by our Board staff. To expedite processing, please submit all required supporting documentation with your application and the requisite fees. Once your application has been deemed complete, your nuclear pharmacist license will be granted; the actual copy of your license should arrive in the mail within 2 weeks of the issuance date.
